Student Employee Payroll Forms

The following forms are REQUIRED before beginning work:

• I-9 Employment Eligibility Vertfication  - Once completed, please print only page 4 and bring to the Student Employment Office.
• Federal W-4 Form
• Michigan MI-W4 Form
• City of Muskegon - MW-4
• Direct Deposit
• Student Employee Confidentiality Agreement

The only page that you need to print to complete is the City of Muskegon page.  The rest can be filled out online, printed, and then signed.
